Requestor must complete a facility request form

• Provide proof of general liability insurance specific to rental request
• Must include set up, decorating and breakdown times in rental request
• Payment for rental is required for appoval of rental (Rentals are on a first come, first served basis)
• Full payment is required 2 weeks prior to rental
